Mohs Surgery and Reconstruction
The Mohs technique of skin cancer excision uses serial sectioning, with 3 – dimensional tracing of the entire tumor, to minimize the risk of cancer recurrence. The lateral and deep margins of the tumor excision are examined to verify that the tumor is completely removed. It is used for “difficult” skin cancers, categorized as those which lie in critical anatomic locations (about the eyes, ears, or nose), for cancers large in size, and for recurrent cancers, aggressive and infiltrating cancer or morphea or sclerosing cancer types.
The high cure rate associated with this technique justifies early reconstruction and repair. Smaller defects are often repaired with simple suture closure, often by the Mohs dermatologist.
However, because of the extensive nature of some skin cancers, the defect which follows excision may be large with an unpredictable size and shape due to the unexpected extent of cancer spread. The borders of the wound may be irregular and scalloped. Or if the cancer had been previously treated and has recurred, previous failed attempts at excision and repair may have created secondary deformities (distortions of the remaining nose) that require additional correction or make future correction more difficult. Often, because of the site and size of the wound and because of a past history of recurrence with previous failed reconstructions, the patient is anxious. Fortunately, he or she can be reassured. Modern plastic surgical techniques are very effective in rebuilding the face and a normal appearance. We see many patients with simple and complex wounds after Mohs excision. After a detailed discussion of the available options for repair, we can formulate a plan to correct the problem. Many of the methods used in this office were developed by Dr Menick and he has taught them in publications and lectures throughout the United States and abroad.
